In the wake of Jeanine's demise, Evelyn (Naomi Watts) seizes control of Chicago, initiating a purge of Jeanine's former associates and sealing the city's gates against the newly discovered outside world. Tris (Shailene Woodley) and Four (Theo James) witness the initial trials, with Max (Mekhi Pfeiffer) being the first to face judgment. Evelyn's loyalists clamor for Max's execution, while Johanna's (Octavia Spencer) faction advocates for leniency. Under the influence of Candor's truth serum administered by Kang (Daniel Dae Kim), Max displays no remorse for his past actions, likening humanity to sheep easily led to slaughter. Evelyn, heeding the public's sentiment, permits Edgar (Jonny Weston) to execute Max. Caleb (Ansel Elgort) is next, apprehensive about his fate. Johanna and her followers are met with jeers for their dissent from Evelyn's verdicts.
Four publicly challenges his mother's authority, arguing that replacing Jeanine with Evelyn offers no true progress. Tris and Four conspire to rescue Caleb. Four confronts Evelyn's guards, demanding Caleb's release. He leads Caleb outside, pushes him into a ditch, and stages a mock execution, prompting the guards to depart. Caleb emerges from hiding, and the trio, joined by Christina (Zoe Kravitz) and Tori (Maggie Q), plan their escape from the city. Peter (Miles Teller) joins them, seeking to avoid betrayal. After evading Evelyn's guards, their path to the wall is compromised when a guard alerts Edgar to their intentions.
As they reach the wall, the group deploys grappling hooks to ascend. However, the fence is electrified. Tris descends the wall, with Four following, to disable the electrical box. Edgar and his men arrive, opening fire. Four holds them at bay while Tris plants a bomb on the box. As they retreat, Tris detonates the bomb, destroying the box and eliminating some of Edgar's men. Tris and Four scramble back up the wall. Tori is fatally shot by Edgar, forcing Tris to abandon her fallen friend.
Beyond the city's perimeter, they discover a desolate, toxic landscape scarred by past devastation, with the rain falling in a crimson hue. Edgar's van appears overhead. They flee, reaching a point where a hidden opening reveals a fleet of vans and aircraft. They are taken aboard, and Edgar's pursuing vehicle is destroyed.
The survivors are transported in plasma bubbles to the Bureau of Genetic Welfare. After decontamination, they receive unique barcode tattoos. A video presentation explains the world's downfall: a well-intentioned miracle drug spiraled out of control, leading to global chaos. The Bureau was formed by survivors to rectify the situation.
Tris meets Bureau director David (Jeff Daniels), who hails her as a "miracle" due to her "pure" genetic makeup, contrasting her with "damaged" individuals like Caleb and Four. David reveals he has been monitoring Chicago, explaining his knowledge of Tris's past and current events. He provides Tris with a device to access her mother Natalie's (Ashley Judd) memories, revealing Natalie's childhood in The Fringe, a dangerous area inhabited by infected survivors, and her subsequent role in establishing the Faction system alongside David.
Four is trained by Nita (Nadia Hilker) in drone combat. Peter and Caleb are assigned to surveillance, observing the city. Caleb witnesses Evelyn and Johanna, now leading a faction called the Allegiant, preparing for war. Edgar instigates the conflict by shooting one of Johanna's followers.
During a mission into The Fringe, Four and his team inadvertently kill the father of a young boy. The boy and other survivors are brought in for decontamination. Christina notices the boy has forgotten his father and his own name, suggesting the decontamination process is a form of memory erasure.
David escorts Tris to Providence, a pristine, dystopian city inhabited by the elite. Tris addresses a council, rejecting David's plan to cleanse the world based on genetic purity and damage.
Four is seemingly sent to The Fringe with Matthew (Bill Skarsgard), a Bureau operative who reveals David's true intentions. Four fights off pursuers and crashes their ship, escaping. He instructs Matthew to tell Tris and David that he abandoned him, a lie Tris will recognize. Four is subsequently recaptured.
David dispatches Peter back to Chicago with a memory-erasing gas, presenting himself as a friend to Evelyn. Peter suggests targeting the Allegiant to maintain Evelyn's control. Evelyn tests the gas on her ex-husband Marcus (Ray Stevenson), causing his memories and identity to deteriorate.
Sensing impending danger, Tris commandeers David's hovercraft, accompanied by Caleb and Christina. Nita, harboring animosity towards David, provides them with weapons. They escape, pursued by David's forces who attempt a gentle capture. Tris manually pilots the craft, narrowly avoiding a crash as they breach the camouflage wall.
Tris and Christina infiltrate Evelyn's facility as she prepares to release the gas, unaware of its city-wide impact. Tris reunites with Four, while Caleb maintains external surveillance. As Evelyn initiates the gas release, Four warns her of its effect on both of them, including his own memory. Evelyn hesitates, but Peter shoots her, allowing the gas to spread. Peter frantically tries to stop it but is overwhelmed.
The gas begins to affect the Allegiant. Tris races to the control room, where David attempts to thwart her. Using drones, Tris locates the gas's power grid and destroys it, halting its spread. David expresses his defeat.
Tris broadcasts a message of unity to the city, declaring that genetic status is irrelevant and urging them to stand together against their oppressors. As Tris leans on Four, believing the crisis averted, David continues to observe, plotting his next move.
