This personal drama unfolds as Dopinder, under duress or perhaps misguided loyalty, deposits Wade in the middle of a bridge, a setup for a violent confrontation.
Wade, now in his titular persona, addresses the audience directly, offering a meta-commentary on his own cinematic existence, even joking about a rather intimate encounter with Wolverine to secure his own film. His targets arrive in a convoy of vans. With a characteristic blend of brutal efficiency and dark humor, Deadpool launches his assault, leaping into one of the vehicles and systematically dismantling its occupants. The ensuing chaos results in a spectacular pile-up, eliminating several of his intended quarry. His primary objective, however, is Ajax (Ed Skrein), the enigmatic figure responsible for Wade’s transformation and disfigurement. The bridge becomes a scene of carnage, with one unfortunate henchman meeting a grisly end via a chain, and another splattered against a highway sign.
Meanwhile, at the prestigious Xavier's School For Gifted Youngsters, the mayhem is observed on a news report by the stoic Colossus (Stefan Kapicic). He enlists the aid of the formidable Negasonic Teenage Warhead (Brianna Hildebrand) to apprehend the destructive mercenary.
Deadpool, momentarily hampered by his oversight in forgetting his extensive arsenal and now facing his adversaries with a limited ammunition count, meticulously counts his shots. Despite a few misfires, his deadly accuracy is undeniable, managing to dispatch three foes with a single bullet. The final thug, armed with dual knives, is swiftly impaled by Deadpool. He then pauses to emphasize the unconventional nature of this superhero narrative, signaling the need to rewind the timeline to fully comprehend his journey.
The film then flashes back to a year prior, introducing Wade Wilson as a mercenary operating in New York City. His assignments are often morally ambiguous, such as intervening in a pizza delivery boy's unwanted advances towards a young woman. Despite being hailed as a hero by his client, Wade humbly deflects the title, underscoring his self-awareness.
Wade frequents a bar owned by his friend Weasel (T.J. Miller). It's a place where patrons engage in a grim form of entertainment, betting on the demise of others in a game dubbed the "dead pool." It is within this establishment that Wade encounters Vanessa (Morena Baccarin), a prostitute with whom he shares a profound connection. Their initial bond, forged over shared experiences of unconventional lives, blossoms into a passionate and ultimately romantic relationship.
This period of genuine happiness is shattered by a devastating diagnosis: Wade is afflicted with aggressive cancer, affecting multiple organs. His primary concern shifts from his own mortality to the devastating impact his illness would have on Vanessa. At the bar, Weasel directs Wade's attention to a mysterious figure in the corner – The Recruiter (Jed Rees). This individual offers Wade a lifeline, presenting an opportunity for a cure and the acquisition of extraordinary abilities through a clandestine facility. Despite his reservations, Wade accepts the card, a pivotal decision that sets him on a path of irreversible change.
Returning to the present, Deadpool intercepts Ajax, who is en route on a motorcycle. A swift and precise katana throw sends Ajax crashing. Deadpool impales him with his second blade, but Ajax’s mutant physiology renders him impervious to a fatal blow. In a moment of raw vulnerability, Deadpool removes his mask, revealing his scarred visage to Ajax, a stark reminder of their shared past. Colossus and Negasonic arrive on the scene. Colossus, dismayed by the collateral damage, attempts to recruit Deadpool into the X-Men, an offer he vehemently rejects. Seizing an opportunity while Deadpool’s back is turned, Ajax makes his escape. In a fit of rage, Deadpool lashes out at Colossus, only to suffer broken hands and a fractured foot. Colossus restrains him by cuffing him to his wrist, but Deadpool, in a display of his extreme resilience and defiance, severs his own hand and escapes in a garbage truck, leaving his severed appendage to offer Colossus a final, defiant gesture.
The narrative again shifts to the past, detailing Wade's agonizing decision to undergo the experimental treatment, a choice made for Vanessa's sake, which necessitated his abrupt departure from her life. At the facility, he first encounters Ajax and his formidable associate, Angel Dust (Gina Carano). Ajax explains that Wade's cells require extreme stress to activate latent mutant abilities, a process that will facilitate healing.
Wade endures a series of brutal torture techniques. Ajax reveals his true intentions: to transform Wade into a super-powered slave for auction. Wade’s defiance surfaces when he learns Ajax’s real name is Francis, a detail he mocks relentlessly. Ajax then subjects Wade to a chamber designed to deprive him of oxygen, a process that causes his skin to blister and his face to become severely disfigured. Upon his release, Ajax informs Wade that his cancer has been eradicated. In retaliation, Wade headbutts Angel, retrieves a match from her mouth, and ignites it near an oxygen vent, triggering a massive explosion. As the facility crumbles, Wade confronts Ajax, his newfound strength evident. However, Ajax gains the upper hand, impaling Wade with a metal bar, which he leaves bent in his flesh, and departs as the structure collapses around him.
Wade awakens, alive but deeply disturbed by his altered appearance. He navigates the streets, his face concealed by a hood, but the revulsion of passersby reinforces his fear of Vanessa’s reaction. He resolves to stay away from her, believing his disfigurement would be unbearable for her to witness.
A visit to Weasel confirms his friend's shock and disgust at his appearance, leading to a barrage of cruel jokes. Weasel, however, encourages Wade to create a disguise and pursue Ajax, hoping to find a way to reverse his disfigurement. Embracing the persona of "Deadpool," Wade embarks on a relentless quest to locate Ajax, systematically eliminating his henchmen. He confronts The Recruiter, who divulges information about Ajax's whereabouts before meeting his end at Deadpool's hands. During this period of intense pursuit, Wade finds solace and companionship with Al (Leslie Uggams), a blind woman.
The film then returns to the present, with Deadpool arriving back at his and Al's residence via the garbage truck, his severed hand slowly regenerating.
Meanwhile, Ajax and Angel Dust, seeking to locate Deadpool, infiltrate Weasel's bar. Angel Dust’s aggressive interrogation of Weasel is met with defiance from the bar patrons, who draw their weapons. They discover a photograph of Wade and Vanessa, prompting their departure.
Despite his lingering fear, Wade ventures to the strip club where Vanessa works, with Stan Lee making a cameo as the DJ. He hesitates to confront her directly. A co-worker informs Vanessa that an ex-boyfriend is waiting outside. Believing it to be Wade, she goes out, only to be confronted by Ajax and Angel Dust, who abduct her. Wade emerges to find only Vanessa's handbag, confirming her kidnapping.
Consumed by rage, Wade gathers his extensive arsenal and heads to the X-Mansion, seeking the assistance of Colossus and Negasonic Teenage Warhead to rescue Vanessa and stop Ajax. He wryly comments on the mansion's vastness juxtaposed with the studio's apparent budget constraints, limiting the X-Men presence to just two.
The unlikely trio commandeers a taxi driven by Dopinder, who, it is revealed, has kidnapped his cousin. Deadpool feigns outrage but secretly commends Dopinder’s initiative. They track Ajax to a scrapyard. Deadpool charges into the fray, while Colossus and Negasonic engage Angel Dust. Angel Dust overpowers Colossus, but Negasonic unleashes her formidable powers, repelling Angel Dust.
Deadpool carves a path through the remaining goons, using their bodies to spell out "Francis" in a final act of mockery. He then ascends to the aircraft carrier where Vanessa is being held captive, imprisoned in the same chamber that once held Wade. He engages Ajax in a brutal blade duel. Simultaneously, Angel Dust nearly strangles Colossus, but Negasonic intervenes with a powerful blast, throwing Angel Dust off and causing the carrier to destabilize. As the structure begins to collapse, sending freight containers plummeting, Colossus ensures Negasonic and Angel Dust’s safety, while Deadpool prioritizes Vanessa’s secure landing. The carrier then succumbs to gravity and crashes.
Ajax renews his assault on Deadpool, who eventually gains the upper hand, pinning him down. With his gun at Ajax's head, Deadpool demands his face be restored. Ajax, however, cruelly reveals that no cure ever existed. As Deadpool prepares to deliver a fatal shot, Colossus intervenes, launching into a lengthy moralistic speech about sparing one’s enemy. Weary of the sermon, Deadpool simply shoots Ajax in the head, causing Colossus to recoil in disgust. Finally, Deadpool confronts Vanessa, offering a heartfelt apology for his abandonment. She removes his mask, revealing not his scarred face, but a cutout of Hugh Jackman’s visage. Upon seeing his true appearance, she expresses her willingness to eventually adjust. They share a kiss, and Deadpool bids farewell to the audience as the film concludes.
