The world as Peter Parker knows it shatters the moment J. Jonah Jameson, in his inimitable style, broadcasts Peter's secret identity as Spider-Man to the entire city. The revelation throws Peter and his girlfriend, MJ, into immediate peril. As shocked New Yorkers grapple with the news, believing Peter orchestrated the devastating London drone attacks orchestrated by Mysterio, Peter and MJ attempt a desperate escape. Their flight through the city, pursued by swarms of news helicopters, leads them through the labyrinthine subway system, where they finally manage to evade their pursuers.
Returning to the relative safety of the Parker family apartment, they find Aunt May and Happy Hogan in the midst of a breakup. Their personal turmoil is quickly overshadowed by the public exposure of Peter's dual life. While Peter tries to mediate the situation between May and Happy, the relentless presence of news choppers outside their home forces his hand. The authorities, now aware of his identity, demand his compliance. Peter, along with MJ and his best friend Ned, find themselves interrogated for their alleged involvement in unlawful activities. Despite MJ and Ned's staunch defense of Peter, he secures legal representation in the form of the blind lawyer Matt Murdock, who successfully navigates the charges.
Seeking a return to normalcy, Peter finds himself a pariah, hounded by a public divided between viewing him as a hero or a menace. His friends, assuring him they can weather the storm, encourage him to focus on their future. However, their college applications, including their coveted spot at MIT, are all met with rejection, a direct consequence of their association with the now-infamous Peter Parker. Overwhelmed by guilt, Peter feels responsible for derailing his friends' lives.
In a desperate bid to undo the damage, Peter seeks out Doctor Strange at the Sanctum Sanctorum. He implores Strange to reverse time, to erase the public's knowledge of his Spider-Man identity. Strange, however, is hesitant, citing the inherent risks of temporal manipulation, a lesson learned from the Avengers' battle against Thanos. He proposes an alternative: a powerful spell that could rectify the situation. Wong, Strange's sorcerer, voices his grave concerns about the potential catastrophic consequences of such a spell.
Ignoring Wong's warning, Strange leads Peter to a lower level of the Sanctum and begins to cast the spell, intending to make everyone forget Peter is Spider-Man. Mid-incantation, Peter impulsively alters the spell's parameters, wanting MJ, Ned, May, and Happy to retain their memories of him as Spider-Man. This interference destabilizes the magic, creating a temporary rift, which Strange manages to contain. Strange rebukes Peter for his selfish alteration, especially after Peter admits he hadn't even contacted the MIT administrator to advocate for MJ and Ned's applications before requesting the spell.
Peter, with the help of Flash Thompson, who has now positioned himself as Spider-Man's closest confidant, attempts to reach the MIT administrator as she heads to the airport across the Alexander Hamilton Bridge. Navigating a traffic jam, Peter intercepts her. Just as he begins to plead his case, his spider-sense erupts. Mechanical tentacles erupt from beneath the bridge, sending cars and pedestrians into a panic. Dr. Otto Octavius, also known as Doctor Octopus, emerges, confronting Peter. Peter, quickly donning his Spider-Man suit, engages Doc Ock in a fierce battle, attempting to protect the administrator. During the fight, the administrator's car teeters over the edge, but Spider-Man manages to web it to safety. Doc Ock, however, pulls Peter beneath the bridge and rips a piece of nanotech from his suit, which attaches to his tentacles. Remarkably, the suit's programming allows Peter to control the tentacles, which he uses to rescue the administrator. Otto, observing this Peter, realizes he is not the one he knows and blames for his failed reactor experiment. The administrator, witnessing Peter's heroism, assures him that she will advocate for his and his friends' applications.
Suddenly, a cackling figure, Norman Osborn as the Green Goblin, appears, hurling a pumpkin bomb onto the bridge, causing an explosion. Otto recognizes Norman, but before Peter can react, he is pulled through one of Doctor Strange's portals. Strange reveals that he has created interdimensional holding cells within the Sanctum, now housing Otto and Dr. Curt Connors, the Lizard. Strange explains that Peter's tampering with the spell has fractured the multiverse, allowing individuals from other realities who know Peter is Spider-Man to breach into their dimension. He equips Peter with a magical gauntlet designed to capture these interdimensional threats and enlists MJ and Ned to assist from the Sanctum's basement.
While searching for the other interlopers, Otto expresses disbelief at seeing Norman, as he died years before Otto became Doctor Octopus in his own universe. Peter ventures out to a field of electrical towers and encounters Max Dillon, Electro, siphoning energy. Electro attacks Spider-Man with electrical bolts, but Flint Marko, Sandman, intervenes, remembering Peter Parker's past kindness. Peter, however, must explain that he is not the Peter that Flint knows. He then disconnects the charges from the towers, neutralizing Electro and restoring Max to his normal appearance. Peter uses the gauntlet to send Max and Flint to the holding cells.
Norman Osborn, momentarily free from his Goblin persona, smashes his helmet and retreats to the FEAST center where Aunt May volunteers. May calls Peter, informing him of Norman's presence. Norman, appearing disoriented, rambles about his son, Harry. May urges Peter to help him. Meanwhile, Jameson continues his anti-Spider-Man tirade on his show, receiving intel on Peter's location. Peter brings Norman to the Sanctum, where he encounters Otto. Flint notes that both Norman and Otto are deceased in his universe. The Lizard recognizes Max from his past, and Max acknowledges the Lizard's destructive rampage. Strange reappears with the Maquina de Cadavas, a relic containing the botched spell, intended to return the villains to their respective universes.
Upon learning that these villains are fated to die fighting Spider-Man in their own realities, Peter resolves to find an alternative. He snatches the relic from Strange and flees into the city. Strange, attempting to retrieve the relic, separates Peter's spirit from his body, but Peter's reflexes allow him to evade Strange and repossess his physical form. Strange then pulls Spider-Man into the Mirror Dimension, where Peter argues for a less fatal resolution. Recognizing the geometric nature of the Mirror Dimension, Peter manages to trap Strange in webbing, secure the relic, and escape, taking Strange's portal creator with him.
Peter proposes a new plan: to cure the villains of their corruptions before sending them home, thereby saving their lives. He entrusts the relic and the portal creator to MJ and Ned, then takes the villains to his and May's new apartment, where he finds the necessary components for his plan. He reveals a Stark fabricator, but Max is drawn to the arc reactor's energy. With Norman's assistance, Peter develops a chip to give Otto control over his tentacles. Otto returns the nanotech to Peter's suit, and Peter provides Max with a device to drain his electrical power.
Suddenly, Peter's spider-sense alerts him to Norman reverting to the Green Goblin, attempting to sabotage the plan. The Goblin incites Max to embrace his powers and reject the cure, leading Max and Flint to retreat together. The Lizard breaks free from his containment. The Goblin hurls Otto out a window, but he escapes just as Jameson and his crew arrive. Spider-Man battles the Goblin, who smashes him through the building's floors. Aunt May retrieves the Goblin antidote and flees. They confront the Goblin at ground level, but May's attempt to inject him with the antidote fails. The Goblin strikes May with his glider before escaping and bombarding the news crew and officers.
May, gravely injured, stumbles to Peter. He blames himself, regretting not heeding Strange's advice. May reassures him, reminding him of the mantra, "with great power comes great responsibility." As they prepare to leave, May collapses, succumbing to her injuries. Peter stays by her side as she passes. SWAT teams surround the building, firing at Peter, but he escapes. Happy, witnessing May's death, is apprehended. The news reports May's demise, with Jameson again blaming Spider-Man, intensifying Peter's guilt.
MJ and Ned attempt to use the portal creator to locate Peter. Ned succeeds, but the portal brings forth a different Peter Parker (Andrew Garfield). This Spider-Man convinces MJ and Ned of his identity. A subsequent attempt by Ned brings forth yet another Peter Parker (Tobey Maguire). The two Spider-Men meet, with Tobey-Peter explaining his search for this universe's Peter to offer assistance. Since MJ and Ned don't know where to find him, the two Peters deduce he's likely at a place of solace. MJ knows where to find him.
MJ and Ned locate their Peter on a rooftop, consoling him over May's death and his remorse for trying to help the villains. The other Peters arrive, but Tom-Peter, consumed by rage and hatred for the Goblin, is initially unreceptive. Tobey-Peter shares his regret over wanting his Uncle Ben's killer to die, realizing it brought no solace. Andrew-Peter recounts his guilt over failing to save Gwen Stacy. Witnessing his variants' pain, Tom-Peter resolves not to let May's sacrifice be in vain.
The three Peter Parkers, along with MJ and Ned, collaborate on creating cures, bonding and marveling at each other's unique abilities, such as Tobey-Peter's organic web-shooters. Tom-Peter confides in MJ, who reassures him of her unwavering support. He then contacts the Daily Bugle, intending to address New York and pledge to fix the mess he created.
The Spider-Men lure the villains to the Statue of Liberty, which is being adorned with a Captain America shield, while MJ and Ned guard the relic. The Peters reminisce about past battles, including Tobey-Peter's fight with the symbiote and Tom-Peter's confrontation with Thanos, while Andrew-Peter's encounter with the Rhino is met with reassurance of his "amazing" nature. Sandman, Lizard, and Electro appear, engaging the Spider-Men. Despite their efforts to administer the cures, the heroes are repeatedly knocked down. They strategize, targeting specific villains. Tobey-Peter battles Sandman, successfully administering the cure and promising to return Flint home to his daughter. The Lizard pursues MJ and Ned as they flee onto scaffolding. Tom-Peter intercepts the Lizard, reverting him to Curt. Electro proves formidable, but Otto reappears, assisting Andrew-Peter by removing the arc reactor and delivering the cure. Max expresses remorse, and Andrew-Peter forgives him.
The Green Goblin attacks on his glider. Ned frees Strange from the Mirror Dimension, and he witnesses the battle. The Goblin causes the scaffolding to collapse, and MJ falls. Tom-Peter attempts a rescue but is struck by the glider. Andrew-Peter saves MJ, experiencing a moment of redemption for Gwen. Tom-Peter uses a Goblin bomb to disable the glider, crashing it onto the shield. He webs the Goblin down and begins to pummel him in a fury. As Tom-Peter prepares to impale the Goblin with the glider, Tobey-Peter intervenes. The Goblin stabs Tobey-Peter, taunting Tom-Peter about May's death. However, Tom-Peter injects the Goblin with the cure, purging Norman of his persona. Norman, filled with remorse, acknowledges his harm to "his Peter."
A massive multiversal rift opens, threatening to unleash more villains. Strange declares he cannot prevent them from breaching. Peter proposes a final spell: not only will everyone forget he is Spider-Man, but they will forget he ever existed. This includes Strange, MJ, Ned, and Happy, but it's the only way to mend the fractured reality. Peter shares a heartfelt farewell hug with his variants, acknowledging them as brothers, before informing MJ and Ned of his decision. Ned tearfully bids his best friend goodbye. Peter and MJ share a final, loving kiss before he departs. Everyone is returned to their respective universes.
Jameson continues his public condemnation of Spider-Man. Peter now lives in solitude, his existence erased from the memories of those he loves. He visits MJ at her cafe, where she and Ned have both been accepted into MIT. After a brief, poignant exchange with MJ, Peter walks away. He later visits May's grave, where he encounters Happy. When Happy asks how Peter knew May, he replies, "through Spider-Man," and they both reflect on May's profound impact. Peter returns to his apartment, using a police scanner to monitor criminal activity, continuing his duty to protect New York as Spider-Man.
Mid-Credits Scene: Eddie Brock and Venom, having been pulled into this reality, are at a bar. Upon learning about the Avengers, they are sent back to their universe by the spell. However, a sliver of the symbiote remains.
Post-Credits Scene: A trailer for "Doctor Strange in the Multiverse of Madness" showcases Strange, Wong, Scarlet Witch, and America Chavez battling multiversal threats, including Baron Mordo and a dark variant of Strange.
